Please complete this Acadex Module Readiness Request Form to confirm that you are ready to develop your module site in Acadex.

In the form, please indicate whether you would like to develop your Acadex site from scratch or whether you would like content to be migrated from your existing eFundi site.

This form serves as confirmation of your choice. All lecturers are required to complete the form.

Lecturer Details

Email must end with @nwu.ac.za and must not contain numbers.
